The plant digitizes the easiest record instead of the most demanded one. Teams start with the neat production log because it converts cleanly. Then a customer complaint arrives and the trace still is not there. Consequence: the project shows a screen but survives no real test, and management support fades by month four.
Operators are given a system built for the office. A form with 14 fields, in English, on a desktop, twenty metres from the machine. Consequence: entries get batched at shift end from memory, which is paper with extra steps.
Data is collected but nobody owns the red cell. The dashboard goes live, a metric turns red, and no one is accountable for closing it. Consequence: the screen becomes wallpaper within six weeks.
Integration is postponed to "phase two". Floor data and ERP data stay separate, so month-end still needs manual reconciliation. Consequence: finance keeps its own numbers, and the plant's data never becomes the official version.

The key steps involved are:

Map the process as it actually runs, not as the SOP describes it. Watch one full shift. Note every place a number is written, copied or verbally passed.
